This form is an official requirement for the state of NH which provides an emergency medical release and alternative people who may pick up you child.

This policy should be reviewed in full by families before enrolling.  This document covers almost everything you will need to know about our school.

Please complete this form if you are providing sunscreen, bug spray, or diaper rash cream for your child.

NH State Law requires we have documentation of an annual physical and updated records of immunizations. Many physicians have their own print out which will also meet this need.

Children needing medicine throughout the day day at school will need to complete this form and deliver with medicine in original container, prescribing information, and name on label.

Students with asthma  must have this form completed by their physician. It is our guiding tool for understanding your child's symptoms and how to treat them.
